Health Care for Re-Entry Veterans (HCRV) Services and Resources

Helps veterans achieve success and avoid homelessness following release from incarceration in state or federal prison. Provides outreach and assessment services; referral services to connect veterans to clinical, mental health, and social services, such as job assistance; and short-term case management support.

VA Telehealth Clinical Resource Hubs (CRH)

Brings together primary care, mental health, and specialty care clinicians and communities facing challenges through telehealth connections to meet the needs of rural and Native American veterans. Provides increased access to healthcare services across veteran populations facing geographic or social barriers to care. CRH consists of integrated care teams that focus on whole health approaches to treating pain, substance use disorder (SUD), and other mental health conditions.

VA Telephone Lifestyle Coaching Program (TLC)

Provides telephone-based lifestyle coaching to veterans with services focused on areas such as alcohol use, food consumption, weight management, stress reduction, and more.

VA Veteran Readiness and Employment (VR&E)

Offers services designed to help veterans and service members secure employment, including assistance with job training and accommodations, resume building, job skill development coaching, and more. Contains information on vocational counseling and support for military family members, and informs employers about the benefits and possible incentives for hiring veterans.
