Purpose

Provides funding to recruit and train emergency medical services (EMS) personnel in rural areas. Focuses on building the capacity of EMS staff through training on trauma-informed, recovery-based emergency care for rural residents with substance use disorders and co-occurring mental health conditions.

Required activities include:

  • Train EMS personnel on SUD/COD trauma-informed, recovery-based care for people with such disorders in emergency situations
  • Purchase emergency medical services equipment, with approval from SAMHSA
  • Train EMS personnel on the use of naloxone, and/or other Food and Drug Administration (FDA)-approved opioid reversal medications, in emergency opioid overdose situations
  • Provide training and resources, and implement procedures to connect individuals who have undergone overdose reversal to SUD treatment
  • Train EMS personnel on the use of motivational interviewing to engage individuals who use substances, and/or who suffer mental health conditions, and provide referrals to treatment and harm reduction services
  • Train EMS personnel on working with local peer-support specialists to assist those who have undergone overdose reversal, including facilitating warm handoffs
  • Create procedures to ensure all overdoses are reported to the state and/or local public health department so follow up care and connections/referral to treatment and peer recovery support can be provided
  • Train EMS personnel to maintain licenses and certifications relevant to serve in an EMS agency, as appropriate
  • Offer courses that qualify graduates to serve in an EMS agency
  • Fund specific training to meet federal or state licensing or certification requirement

Other allowable activities may include the purchase of the following:

  • Personal protective equipment
  • FDA-approved medication or devices for emergency reversal of known or suspected opioid overdose
  • FDA-approved fentanyl and xylazine test strips
  • Buprenorphine - for use after overdose reversal, to treat opioid withdrawal and bridge individuals into treatment, as directed by a licensed and trained medical practitioner

Award ceiling: $200,000 per year
Project period: Up to 2 years
Estimated number of awards: 52
Estimated total program funding: $10,400,000

A 10% non-federal matching contribution is required. Matching funds can be cash and/or in-kind contributions.

Who Can Apply

Rural emergency medical service agencies operated by a local or tribal government (fire-based and non-fire based) and non-profit emergency medical service agencies are eligible to apply.
