Brings together primary care, mental health, and specialty care clinicians and communities facing challenges through telehealth connections to meet the needs of rural and Native American veterans. Provides increased access to healthcare services across veteran populations facing geographic or social barriers to care. CRH consists of integrated care teams that focus on whole health approaches to treating pain, substance use disorder (SUD), and other mental health conditions.

What This Program Offers

VA Telehealth Clinical Resource Hubs (CRH) provide clinical services through virtual care, in-person care, and mobile teams through a combination of primary care, mental health, pharmacy, nursing, and specialty care providers. CRH can provide:

  • Team-based primary care services
    • Primary care providers
    • Clinical pharmacy specialists
    • Primary care mental health integration
    • Registered nurse care coordination and case management
  • Team-based outpatient general and specialty mental health services for mental health conditions
    • Evidence-based psychotherapy
    • Evidence-based pharmacotherapy

  • Team-based specialty care services
    • Teledermatology
    • Telecardiology
    • Teleneurology
    • Telerheumatology
    • Teleinfectious disease
